If you are a unit buyer or a lender or mortgage broker for a unit buyer of a condominium we insure, you may use the following form to request a certificate of insurance for the Condo Master Policy. The certificate will be sent to the recipient information you provide.

IMPORTANT STATEMENT:
Before you submit your completed request, please verify that all information is correct (ie: spelling, contact information) and that it will be sent to the correct email address recipient. All information entered should appear exactly as it is stated in the loan documents. All second requests will incur a processing fee of $25. Any subsequent requests for the same certificate will incur a $50 charge.

Certificates will be provided within 24 business hours of your request. All "rush requests" will be expedited to the best of our ability.
